Police Record Check
Police Record Check forms (PRC’s) are required for all team officials (Coach, Assistant Coach and Trainer).
When all team representatives have been identified, you will need each team official to complete the required form and provide the proper copies of identification for the police record checks.

Once all team officals have completed their forms you will need to submit the package to your Convenor. Each Convenor has an envelope at the canteen at Blackburn Arena where the package can be left.
The BMHA will cover the fees for the Police checks. Please note that the PRC must be completed every year.
Player Medical Forms
Your Coach will receive the Player Medical Forms that were completed at the time of Registration from you Convenor. If you are missing any forms please contact the BMHA Registrar. These forms should remain with the team manager and trainer in case of emergency during a game or practice.
Official Team List
The BMHA Registrar provides the Official Team List to each team listing all players and team officials. This list must be signed by each person and returned to the Registrar. Copies of this list should be made and taken to each game played (league, tournament or exhibition).
A copy of this list might be requested during tournament registration.
Certification Courses
Follow-up with team officials to ensure certifications are current and appropriate courses have been taken (e.g. Speak Out). All team officials must have at least the Speak-Out course certificate -- manager's included.
The Head Coach must have the Coach Stream / Level I Coaches Certificate and the trainer must attend the Trainer's course. Any courses taken are financially covered by the BMHA. If the course is taken taken outside of the BMHA the participant must pay the registration fee and submit the receipts to the BMHA for reimbusement.
